TERMS AND CONDITIONS

Tandartspraktijk Duinzigt

These terms and conditions apply to all treatment agreements between Tandartspraktijk Duinzigt, located at Waalsdorperweg 449, 2597 KP The Hague, and the patient (hereinafter: “client”).

1. The treatment agreement

1. These terms and conditions are part of the treatment agreement as referred to in Section 7:446 of the Dutch Civil Code (WGBO).

2. The conditions apply to all dental treatments, budgets and appointments.

3. The general terms and conditions are available on the website and are available for inspection at the practice.

4. Tandartspraktijk Duinzigt reserves the right to change these conditions.

2. Execution of the treatment

1. The treatment is carried out in accordance with the standards of care that may be expected of a good healthcare provider and in accordance with applicable laws and regulations and professional guidelines.

2. Dental treatments involve an obligation to make an effort, not an obligation to achieve a result.

3. The practice is entitled to engage third parties if this is necessary for the execution of the treatment.

4. The client is obliged to provide correct and complete information about his/her health condition.

3. Costs and rates

1. Dental treatments are declared according to the rates established by the Dutch Healthcare Authority (NZa).

2. For treatments with an expected total amount of €250 or more, a budget will be provided in advance.

3. If changes are necessary during the treatment, additional costs may be charged after consultation with the client.

4. Technique costs, if applicable, will be charged separately.

4. Appointments and cancellation

1. Appointments can be canceled or rescheduled free of charge up to 24 hours before the scheduled time.

2. In the event of non-cancellation or late cancellation, the reserved treatment time may be charged.

3. The practice reserves the right to change or cancel appointments in case of force majeure.

4. The practitioners at our practice make every effort to keep appointments as much as possible at the scheduled time, but are not liable for delays or waiting times as a result of unforeseen treatment circumstances or emergency care.

5. Payment and invoicing

1. Invoicing can take place directly with the client or via an external invoicing service.

2. In the event of payment arrears, further treatment may be suspended, unless there is an emergency.

6. Complaints

1. Complaints about the treatment or invoicing must be reported to the practice within a reasonable period of time after discovery.

2. Complaints are handled in accordance with the applicable complaints procedure within dentistry.

3. Submitting a complaint does not suspend the payment obligation.

7. Liability

1. Tandartspraktijk Duinzigt is only liable for damage resulting from an attributable shortcoming in the treatment.

2. Liability is limited to what is paid out by the professional liability insurance.

3. The practice is not liable for damage or loss of property of the client.

8. Confidentiality and privacy

1. All information about the client will be treated confidentially.

2. Processing of personal data takes place in accordance with the applicable privacy policy of Tandartspraktijk Duinzigt.

9. Applicable law and disputes

1. Dutch law applies to all treatment agreements.

2. Disputes will be submitted to the competent court in the Netherlands.

10. Final provision

If a provision from these general terms and conditions proves to be void or voidable, the remaining provisions will remain in full force.
